We are recruiting for a Contact Center Coordinator to join Lacuna Health in Louisville, KY. Summary: -
Acts as a focal point for the referrals and admissions. -
Works with internal and external healthcare professionals to coordinate transition of patients -
Performs direct tasks needed to assist Nurse Advocates in screening, coordinating and assisting with the transition and discharge planning process. -
Maintains a current knowledge of Medicare and State Medicaid requirements and practices. -
Acts as Kindred representative in dealing with third-party payers and insurance companies. Essential Functions: -
Customer Service & Satisfaction – Models exemplary customer service to all constituencies at all times. Establishes professional and appropriate repertoire with both external and internal staff. Responsible for facilitation through email and phone with Lacuna Health Liaisons, Nurse Advocates, Central Placement offices, Clinical Liaisons, and all other employees involved in the admissions and discharge process. Maintains appropriate communication with insurance companies when verifying benefits and making other inquiries. Trains new coordinators and facilitates learning through demonstration and teaching. Communicates and collaborates with the Nurse Advocate to relay clinical information and clinical needs. Monitors and evaluates admissions and discharges for assigned facilities. Shares results with the Nurse Advocate, management team, and others as appropriate to develop action plans as needed. -
Systems expertise – Navigates various systems to keep track of patient information and insurance information, including but not limited to, SharePoint, Protouch, Point Click Care, Meditech, and HPAS. Navigates through systems to pull monthly revenue data and creates reports based on this data. -
Specialized Healthcare Industry Expertise – Maintains a working knowledge of Federal and State regulations and reimbursement guidelines (Medicare and Medicaid). Assists Nurse Advocate in determining benefits for third-party payers and other insurance companies. Maintains a working knowledge of the admissions and discharge process. -
Admission Process Paperwork & Documentation – Creating referral packets in accordance with policies and procedures. Ensures accurate information is sent to both internal and external sources. Keeps liaisons up-to-date with referral information for assigned patients in assigned markets. Ensures that information sent is in accordance with patient/family choice. Monitors and facilitates all correspondence with liaison's to facilitate patient discharge. Escalates clinical needs to Nurse Advocate as appropriate. -
Administrative Tasks – Keeps all patient information confidential. Performs other duties as assigned. Knowledge/Skills/Abilities: -
Expert ability to develop relationships with referral sources through the use of all communication channels deemed appropriate. -
Expert ability at collaborating with clinical personnel for a team approach to referral management and discharge planning. -
Proficient user knowledge of Windows Office programs (Word, Excel, PowerPoint), and the ability to learn specialized computer applications (Protouch, Point Click Care, HPAS, Meditech) that are specific to handling job requirements. -
Working knowledge (or the ability to learn) of healthcare reimbursement programs (payer sources). -
Ability to develop professional working relationships and communicate effectively with multiple constituencies –Case Managers, Liaisons, Admissions Coordinators, external personnel, fellow associates, etc. -
Ability to plan, organize and prioritize multiple tasks that need to be completed on a daily basis. Excellent organizational skills and documentation orientation. -
Ability to maintain confidentiality. -
Ability to work independently. Must be self-motivated and goal oriented. -
Ability to communicate effectively, both orally and in writing. -
Ability to work flexible hours, as admission responsibilities may dictate. -
Ability to maintain regular attendance. -

Associate's degree in a healthcare related field. -
Bachelor's degree preferred. -
2 years of equivalent experience in a sales/marketing position or Admissions Coordinator position will be considered in lieu of a degree. License/Certification: - Must have a valid driver's license.
