The Clinical Quality Coordinator shall work in a practice setting closely with and under the direction of the Ambulatory Quality Program Manager, THMG and ultimately, the Director, Ambulatory Clinical Quality, THMG to improve performance by identifying, monitoring, and closing care gaps related to preventative and chronic care conditions, age appropriate screenings and immunizations in support of the quality programs for The Tower Health Medical Group.
This position's overall purpose is to support quality-related aspects for reducing care fragmentation while optimizing patients individual experience of care, improving health of populations, reducing costs of care for populations and enhancing care team satisfaction (Institute of HealthcareImprovement Quadruple Aim).
