DEPARTMENT: Geisinger Health Plan- Care Coordination

Provides clinical / administrative oversight of Medical Home and Care Coordination Case Managers and manages operational oversight of Medical Home implementation within assigned primary care practices. This includes delivery of disease / case management services, population analysis, primary care practice redesign, systems of care coordination, process improvement strategies, barrier analysis and data management. Monitors staff performance and coordinates ongoing staff development strategies. Ensures staff compliance with regulatory requirements. Supports the goals and objectives of Medical Home, Geisinger Health Plan (GHP), and the Geisinger Health System (GHS). Member of the Medical Home Management Team comprised of Director, Case Management and Vice President Health Services. Works under direction of Director Case Management and Vice President Health Services.

EDUCATION AND/OR EXPERIENCE:
Registered nurse licensed in the state of Pennsylvania required.
Minimum five years clinical experience in nursing required.
Bachelor's degree in health care related field preferred.
Experience in disease and case management required.
Leadership or prior supervisory experience preferred.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Independent travel is required.
