The Registered Nurse (RN) is responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran's care. Serve to support delivery of high quality, timely care to the Veteran receiving services. The RN of the Community Living Center serves Veterans needing long-term care, rehabilitation care, hospice care, mental health care and/or respite stay. RN duties include but are not limited to: Completes nursing assessments including the Veterans risk factors; physical, psychosocial, emotional and mental status; medications; environmental and safety and educational needs. Based on the assessment, referrals are made to other members of the IDT which may include Provider, Social Work, Occupational Therapy, Restorative Therapy, Dietitian, and Psychologist. Develops an initial plan of care with interdisciplinary care team (IDT), Veteran and family members. Reviews the interdisciplinary plan of care with all team members during the quarterly team meeting. Collaborates with pharmacist through interdisciplinary team meetings, or as needed based on quarterly pharmacy review of medications. Consults with inpatient discharge coordinators, nurses and social workers to facilitate the transition to home care. Completes treatments as ordered. Serves as a liaison to both internal and external providers and will be responsible for managing the care of the Veteran and the seamless transition to and from the community. Work Schedule: 80 hours every two weeks. This is a Day/Evening rotation (Days: 6:30 a.m. - 3:00 p.m.; Evenings: 2:30 p.m. to 11:00 p.m.) with rotating weekend and holidays.
