The Registered Nurse will work collaboratively with all levels/services of the medical center, associated CBOCs, VA facilities inside and out of VISN 19, and non-VA health care entities who provide care to Veterans. Additionally, this person is accountable to the Associate Director, Patient Care Services for ensuring the Standards of Care and Nursing Practice are met. Responsible for collaborating with services internal or external to the VA to provide care coordination for Veterans requiring primary and specialty services. Process consults utilizing clinical judgement and VA guidelines to determine if services can be provided in-house, via Tele-health or E-consult, inter-facility or if they need to be referred for Care in the Community. Provide patient and family education to facilitate successful participation in accessing health care. Demonstrate leadership by serving as a resource or mentor to VA and non-VA stakeholders to enhance professional growth, development, and practice. Acquire and demonstrate clinical expertise with planning, designing, integrating, implementing, modifying and evaluating the effectiveness of the services as it relates to VA care in the community and/or services to improve the quality of patient care and the appropriateness and timeliness of services provided. Provide the clinical guidance and expertise for the coordination of the various care options available to the Veteran by providing clerical, administrative and financial management support for medical services under the Veterans Medical Benefit Package. Organize and coordinate the clinical components for the authorization of services in response to individual healthcare needs in multiple settings while centering services around the patient to foster patient self-managed care and maximize efficient and cost-effective use of health care resources. Utilize all avenues of resources available to provide safe and efficient timely coordination of quality health care and services. Provide technical, administrative and logistical support to Veterans and their family members, VA staff, and non-VA health care providers to effectively manage the care of Veterans. Duties will be varied. Primary responsibility is to utilize various options available for providing care to the Veteran (i.e. in house consultation, Inter-facility Consults (IFC), and non-VA purchased community care) per current protocol and within VHA's timeliness standard. Coordinate services with PACT and external stakeholders to facilitate coordination of care to ensure timely access to healthcare services. Facilitate and develop strong professional relationships with VA and external customers, including community providers, services, local area facilities, and Third Party Administrators (TPA). Serve as a liaison to community providers to facilitate and support Veteran access to care and high complex care needs, providing education on community care processes as needed. MTVAHCS and the RN embrace patient centered care principles. Require frequent contact with non-VA healthcare facilities and providers, handling of sensitive data and medical documents. Adept (or able to be trained) in CPRS, HSRM, PPMS, TPA portals, and other pertinent electronic programs . Function as the initial contact for providers inquiring about consults, authorizations, processes, and procedures. Collaborate with several disciplines (i.e. VA and non-VA staff, IFC, specialty services, and the Patient Business Office). Support a global perspective in prioritizing work and assisting team function. Leadership and nursing skills are provided in a holistic approach. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses).
