This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The responsibilities for the RN Case Manager include, but are not limited to the following: Provides initial and ongoing assessment of patients to identify needs, care goals, and appropriate resources necessary for case management. Demonstrates knowledge in using VA Medical Center and community resources to provide the highest quality of care in the most cost-efficient settings. Provides leadership in the application of the nursing process and identifies resources and critical factors for achieving desired outcomes for discharge, post-hospitalization recovery, and health maintenance/improvement. Possesses thorough knowledge of utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. Accountable for utilizing critical thinking skills and sound judgment to ensure positive outcomes and delivering quality patient care. Demonstrates knowledge and ability to apply principles of evidence-based practice. Provides support at the unit level, the service level, and the organizational level.
