Provides leadership in the application of the nursing process to patient care, organizational processes and/or systems, improving outcomes at the program or service level in the Caregivers Support Program. The incumbent will function in support of caregivers, Veterans and family members throughout the medical center and in community settings. The incumbent will conduct initial and annual home visits to Family Caregivers approved in the program and will conduct the Veteran Functional Assessment Instrument (VFAI) and other assessments as needed via phone and clinical video telehealth. The incumbent is responsible for developing and establishing linkages and collaborate on VA caregiver support efforts with community healthcare and social service agencies. The incumbent possesses advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources. The incumbent demonstrates proficiency in practice based on conscious, deliberate planning. Self-directed in goal setting for managing complex patient situations. The Clinical Nurse Assessor participates in face-to-face or virtual contact with the caregiver and Veteran to assess function and the need for personal care services. As a member of the interdisciplinary team, they will participate in the development and application of directed clinical treatment interventions. The Clinical Nurse is to be responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
