The Care Coordinator is administratively, professionally, and clinically responsible to the Chief of Community Care. Pay Plan NG42 The RN Community Care Coordinator responsibilities include short and long-range planning, analyzing and interpreting laws, regulations and policies which lead to the development and implementation of new procedures and methods of operation, and quality monitoring of performance measures. Collaborates effectively with program staff, Primary Care Providers, other healthcare professionals and team members, other clinics, internal and external services, and community agencies to manage the established Veteran population. Competent in the field and utilizes his/her ability to support other clinicians working in Community Care with knowledge enhancements and with concerns and/or issues as they arise.
