The Clinic Coordinator plans, integrates, implements, modifies and evaluates the effectiveness of the Veteran's care. He/she provides clinical leadership and expertise on processes and regulations. Serves as a liaison to both internal and external providers and will be responsible for managing the care of the Veteran and the seamless transition to and from the community. This position is for a Clinic Coordinator for the Ambulatory Surgery Center Duties include but are not limited to: Provides a full range of nursing care to patients with complex care needs. Accurately documents in the medical record patient findings, assessments, and care provided. Identifies potential and actual problems involving resources and/or safety. Effectively plans patient care and nursing resources for maximum staff productivity, staff and time efficiency, and positive patient outcomes Educates colleagues and/or students and serves as a preceptor. Initiates/participates in quality improvement activities that result in approved outcomes Work Schedule: Monday thru Friday; 7:30 a.m. to 4:00 p.m. Financial Disclosure Report: Not required
