The Licensed Practical Nurse (LPN) has the knowledge and skills necessary for concentrated effort and self-direction in carrying out the most complex nursing care assignments for selected patients requiring skilled nursing care on a regular/recurring basis. The LPN has a broad work experience with demonstrated skills sufficient to resolve a full range of nursing and patient care problems. The incumbent will perform a variety of nursing care assignments within the Patient Aligned Care Team (PACT). Completed work needs only a general review by a registered nurse for technical soundness, appropriateness and conformity with policy and nursing assignments. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN is a member of the Patient Aligned Care Team (PACT). The LPN demonstrates active collaboration and communication with the Teamlet and may direct other members of the Teamlet in providing certain aspects of care for a designated panel of patients (Patient, Primary Care Provider, RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patient).
