Your day.
As an RN Discharge Navigator you need to know how to:
- Coordinate and direct patient care between units/departments.
- Coordinate efficient and effective care between disciplines.
- Stage patients for discharge through the Care Team.
- Follow-up on patient and family questions and concerns regarding their medical condition post-discharge.
- Actively engage and drive rounds on the units.
- Identify resources and roadblocks to insure more efficient patient flow and timely care and discharge.
- Question and prevent duplication, fragmentation, or use of unnecessary resources.
Your experience.
We hire people, not resumes. But we also expect excellence, which is why we require:
- ASN from an accredited program with the ability to obtain your BSN from an accredited program within 4 years of your hire, required
- Current license as a Registered Nurse in the state of practice, required
- Current Basic Life Support (BLS) course completion card (ACLS/NRP/PALS as appropriate for the unit of service), required
- Minimum of three (3) years of nursing experience, required
- Masters of Nursing Degree (MSN), preferred
- Previous nursing leadership experience in a Charge Nurse or Relief Charge Nurse role, preferred
